Can I Change My Daughter's Name?

After my divorce my last name went back to my maiden name. My daughter (who is 12) would like to add my last name to her name. She would still keep her father's last name. She wants to add it as part of her middle name. How do we go about this?

A. 

This varies by state. My experience is that most states will allow the change of a child's name (other than last name) with the approval of one parent, but you should check in your state to see what specific rules apply there.
